What to Expect

RECEPTION

Our reception staff is always waiting to welcome you in a, professional, friendly manner. On arriving to the clinic you will be asked to sign-in at the front desk.

CHECK IN

Our Check-In process is smooth and efficient. We see patients without requiring an appointment and typically, in the order they sign in unless they have an emergency that is life threatening (this is up to the Physicians discretion). Rest assured we are aware that your time is valuable and we are working hard to minimize your wait. Our intention is to limit your visits to less than one hour.

New patients – you will be asked to review our Privacy Policy and to complete a Sign-in form, Protected Health Information Form (this authorizes us to contact you with any relevant follow-up information), and Acknowledgement of receipt of Privacy of Practices Form.

Existing patients – who have not been seen at the clinic for over a year, or who have new insurance or new contact information will also be required to complete these forms.

You may print these forms out and bring them in completed to save check in time (if you have any questions we will be happy to help you fill out the forms once you arrive).

After completing the forms our receptionist will:

Photocopy your ID and Insurance Card and;

Collect your Co-pay or charge your deposit

TRIAGE

Following sign in, your visit will consist of, a medical assistant will take a brief history, medication list, reason for your visit and get your vital signs. This important step provides a baseline of information that will assist our providers in diagnosing your ailment and providing the best treatment options.

PROVIDER EXPERIENCE

Once you have been triaged by one of our medical assistant, you’ll be cared for by one of our, experienced licensed physicians. The physician will then review the triage notes and ask additional questions about your health or reason for your visit, and will examine you to determine the proper diagnosis.

Additional tests such as X-rays, blood tests and EKGs, may be performed as necessary. The physician will then discuss any lab or x-ray results with you and answer all of your questions. We encourage you to ask any questions you may have about your tests or diagnosis.

Discharge Instructions – You will be provided with information regarding our visit, the physician’s treatment recommendations, and follow-up instructions. You will receive a copy of these instructions for your records and any prescriptions for any medications prescribed on discharge at the front desk.

You may print these forms out and bring them in completed to save check in time (if you have any questions we will be happy to help you fill out the forms once you arrive).

If a condition requires a test or additional treatment, our Physician will let patients know the cost ahead of time. When patients check-out, they will know exactly how much the entire bill is — with no additional fees that pop up months after a visit.
